The survey looks at everything to do with supporting an SPL club, the game itself, match facilities and supporters clubs. It only takes five minutes to complete the survey - https://docs.google.com/spreadsheet/viewform?formkey=dHFQWGN6Q0htYmxWRnFjRTdaU1FyVnc6MQ#gid=0 The results of the survey will be used to pull together an extensive report on the state of the SPL which will be available from the end of January 2013.
